Disability Verification Forms

Students seeking disability accommodations under the law must have adequate documentation on file in the Disability Services office in a timely manner before accommodations can begin. Documentation must be from a specialized, credentialed authority who can address the functional limitations due to the disability and its impact on the academic or residential setting. Under the law within a postsecondary setting, a student does not qualify for services until they have registered with Disability Services and have been certified for eligibility. Retroactive accommodations are never made.

In order to assist you in obtaining appropriate documentation information, please use the following forms. Choose the appropriate form and have your qualified health care provider complete it. Return the forms to Disability Services as directed on the form as soon as possible in order to receive reasonable accommodations in a timely manner.

*Please note that learning disability documentation must not be over five years old, ADHD documentation must not be over three years old, and psychological disability documentation must not be older than one year.
