MMR - Measles, Mumps, Rubella
Two doses:

• 1st after 12 months of age
• 2nd booster before school (usually kindergarden)
  If birthday is before 1959 documentation of having measles is needed. If no documentation is provided 1 MMR is required.

TETANUS
Required documentation of immunization within 10 years of entrance to college.  Tdap is highly recommended. 

TB
PPD Mantoux is required within 1 year of entrance to college.
If (+) result, you will need documentation of a negative chest x-ray within the current year and documentation if you have had INH therapy.

HEPATITIS B
Documentation of the first two doses in the series of three.

POLIO
Documentation of primary series completed and date of booster dose.

MENINGITIS-- One dose is required.

OTHERS
If immunizations are omitted due to religious reasons, documentation is required. TB screening is still required.

Returning students will need an updated physical and current PPD Mantoux if absence from school has been > 2years.

A current PPD Mantoux is required for students traveling abroad during a previous semester.
