Human Resources - Prescription Plan
Employees who are enrolled in one of the three medical insurance plans are also provided with the following prescription benefit.
Retail
There is a $25 deductible per year. Coinsurance is 25% for generic and brand preferred prescriptions, and it is 45% for brand non-preferred prescriptions.
Mail Order
The cost for mail order prescriptions is as follows:
$15.00 for generic prescriptions
$30.00 for brand preferred prescriptions
$45.00 for brand non-preferred prescriptions

There is a generic and mail order mandate. This requires all maintenance prescriptions to be filled through mail order. Please contact Human Resources at extension 1406 for the proper forms or if you have any questions.
