How do I get a copy of my health record or have it sent to my new school?

All written/typed requests for health records must be sent/faxed to the Health Center at:

Requests MUST include:

  • Your full name, address and phone number
  • Date of Birth, SS#
  • Exact records needed
  • Exactly where records should be sent or faxed
  • Processing fee of $5.00 with checks made out to Elizabethtown College

When is the submission deadline for first year students' health records who are starting in the fall?

Health records should be sent to Health Services by August 1st of the entrance year. If this is not possible, please notify Health Services.
. It is important to submit your Health Record for review as soon as possible to resolve any incomplete documentation. Physicals and TB mantoux screenings must be current within the year of entrance.

 

Can I receive immunizations once on campus if I can't get them at home?

All immunizations, including the meningitis, are available at the Health Center in the fall 2005 semester.

 

INSURANCE

Do I need the insurance policy provided by the college, or will my own cover me?

Every insurance policy is different and it will be important to investigate your policy requirements/constraints prior to arrival at college. The student policy offered through Elizabethtown is a good supplemental accident and illness policy. Details of the policy can be reviewed at the following web site:

All students receive care regardless of insurance choices.

There are limited costs at health services which are minimal.

All international students are required to carry the college policy.

A free discounted vision plan is available through the Health Center for a local eye care center. Enrollment is done after arrival to campus by stopping in the Health Center.

There is no separate dental policy for students.


If I have the college insurance policy can I seek care over the summer?

The college policy is current until August 15th of the purchased year. Care administered outside of the Health Center during the summer months (Health Center is closed from June 30th to July 31st) will incur $50.00 per incident. A claim form is needed to submit claims and can be obtained from Bollinger's web site or by calling the Health Center at (717) 361-1169.

How can I get my routine medications while at college?

A local pharmacy to delivers prescription medications on a daily basis as needed (except weekends). Sloan's Pharmacy can easily transfer most prescriptions to their service and work with a wide variety of insurances. To verify insurance coverage call (717) 653-6888. This allows easy access to routine medications, students simply phone when a medication is due for reorder. Sloan's will deliver the medication to the Health Center for convenient pick up. They will bill the Health Center according to insurance guidelines, and the Health Center will in turn place this charge on the student statement. This service is provided free of charge to students.
